Registered Nurse - Weekends

Job Summary

A company is looking for a Registered Nurse - Weekends (Remote).

Key Responsibilities:

• Plan and conduct intervention opportunity evaluations, respond to urgent alerts and remote patient monitoring alerts
• Work directly with patients to develop and achieve patient-centered chronic care management goals
• Collaborate with engagement and product teams to promote quality outcomes and optimize service experience

Required Qualifications:

• Compact nursing license
• New York nursing license preferred
• At least 2 years of nursing experience providing care to adult and geriatric patient populations
• Graduated from an accredited nursing program
• Confidence with clinical skills in performance of telephonic triage/assessment
