Registered Pharmacist Population Health

Job Summary

To support the development and administration of clinical programs, the full-time remote Registered Pharmacist, Population Health will manage program implementation, outcomes reporting, and collaborate across departments to enhance patient care.

Key responsibilities

• Lead the development and maintenance of clinical programs while facilitating communication and education to stakeholders
• Provide clinical outreach to enhance care coordination among members, prescribers, and pharmacy providers
• Support operational departments with the implementation of PBM programs and ensure compliance with industry standards and regulations

Required qualifications

• Bachelor's degree in Pharmacy or PharmD required
• Current, unrestricted licensure as a Registered Pharmacist in the state of residence
• 2+ years of clinical experience or completion of a clinical or managed care pharmacy practice residency with 1 year of clinical experience
• Knowledge of PBM industry principles and formulary management preferred
• Proficiency in electronic databases and MS Office applications preferred
