Job Summary

A non-profit Catholic-based healthcare company needs applicants for an opening for a Remote Advance Care Planning Coordinator.

Candidates will be responsible for the following:

  • Assist clinical staff in for preparing patient appointments (pre-visit planning)
  • Provide community based support and assistance to patients and staff
  • Act as a resource to patients and families, providing answers to frequently asked questions

Applicants must meet the following qualifications:

  • May be required to occasionally work irregular hours, attend other job related meetings
  • Required Minimum Education: GED
  • 1 year experience in community care, ambulatory care, or inpatient care setting
  • Excellent interpersonal communication skills
  • Experience with data entry and tracking
  • Strong analytical, data management and computer skills
