Job Summary
A non-profit Catholic-based healthcare company needs applicants for an opening for a Remote Advance Care Planning Coordinator.
Candidates will be responsible for the following:
- Assist clinical staff in for preparing patient appointments (pre-visit planning)
- Provide community based support and assistance to patients and staff
- Act as a resource to patients and families, providing answers to frequently asked questions
Applicants must meet the following qualifications:
- May be required to occasionally work irregular hours, attend other job related meetings
- Required Minimum Education: GED
- 1 year experience in community care, ambulatory care, or inpatient care setting
- Excellent interpersonal communication skills
- Experience with data entry and tracking
- Strong analytical, data management and computer skills
