Job Summary
To support the dementia care program, the part-time Remote Advanced Practice Provider will review clinical documentation, complete required attestations, and collaborate with clinical teams to ensure high-quality care for patients and caregivers.
Key Responsibilities
- Review and sign off on documentation from care navigators and nurses in a timely manner
- Conduct remote calls and video visits, including urgent escalations, while maintaining accurate patient records
- Provide oversight and support to care navigators and nurses involved in the dementia care program
Required Qualifications
- Graduate of an accredited school of nursing
- Minimum 2 years of direct patient care as an Advanced Practice Provider
- Current APRN license(s) with a minimum of 10 independent state practice licenses
- Dementia proficiency meeting at least one CMS criterion
- Proficiency with electronic health records and telehealth tools
