Remote Advanced Practice Provider (NP/PA)

Job Summary

Providing holistic and empathetic care through digital channels, the full-time Remote Advanced Practice Provider (NP/PA) will manage patient interactions, oversee medical management, and collaborate with healthcare teams to enhance patient experience and outcomes.

Key responsibilities

• Deliver exceptional clinical care via real-time and asynchronous digital channels while managing multiple patient conversations
• Guide patients' medical management using a holistic framework and ensure continuity of care through collaboration with primary care providers and specialists
• Maintain accurate digital documentation and identify opportunities for improving patient experience and outcomes within digital workflows

Required qualifications

• Three or more years of experience in primary care, internal medicine, family medicine, geriatrics, palliative care, or urgent care
• Active and in-good-standing RN and APRN licenses for Nurse Practitioners or an active and in-good-standing Physician Assistant license
• Ability to review AI-generated responses for clinical accuracy and safety
• Willingness to obtain licensure in additional states as needed within 90 days of hire
• Prior virtual care or telehealth experience is preferred
