Job Summary
A healthcare, life insurance, accident insurance, and Medicaid and Medicare service provider needs applicants for an opening for a Remote Alegis Chronic Care Social Worker.
Individual must be able to fulfill the following responsibilities:
- Collaborate with the care team to achieve identified patient outcomes
- Identify high-risk/high-cost patients for possible case management and social service intervention
- Initiate contact with patient/family, physician, and health care providers/suppliers to discuss the alternative treatment plan
Applicants must meet the following qualifications:
- Current Licensure as a LCSW-C in the state of practice or where Alegis Care has a chronic care footprint
- Three to five years recent experience in a home care environment or social services environment
- Knowledge of managed care and utilization review requirements and procedures
- Demonstrated ability to coach and educate other staff
- Demonstrate effective organizational skills and flexibility to meet the business needs
- Excellent time management, organizational, research, analytical, negotiation, communication (oral and written) and interpersonal skills
