Job Summary

A non-profit healthcare system has an open position for a Remote Ambulatory Care Coordination Educator.

Candidates will be responsible for the following:

• Monitoring monthly process and outcome measures demonstrating improvement in performance targets
• Creating and maintaining workflow processes/procedures facilitating quality content development
• Identifying strategic opportunities that will positively impact quality of care

Position Requirements Include:

• Bachelor’s degree in a healthcare related field
• RN or LSW
• 5 years of post-licensure experience in nursing or social work with a minimum of 3 of those years in case management
• Strong personal computer skills (Word, Excel, PowerPoint, Visio)
• Excellent verbal, written, and presentation skills
• Demonstrates critical thinking skills
