Job Summary
A non-profit healthcare system has an open position for a Remote Ambulatory Care Coordination Educator.
Candidates will be responsible for the following:
- Monitoring monthly process and outcome measures demonstrating improvement in performance targets
- Creating and maintaining workflow processes/procedures facilitating quality content development
- Identifying strategic opportunities that will positively impact quality of care
Position Requirements Include:
- Bachelor’s degree in a healthcare related field
- RN or LSW
- 5 years of post-licensure experience in nursing or social work with a minimum of 3 of those years in case management
- Strong personal computer skills (Word, Excel, PowerPoint, Visio)
- Excellent verbal, written, and presentation skills
- Demonstrates critical thinking skills
