Remote APRN Provider

Job Summary

To support the dementia care program, the part-time Remote Advanced Practice Provider will review clinical documentation, complete required attestations, and provide oversight to care navigators and nurses while working in a fully remote capacity.

Key Responsibilities

• Review and sign off on documentation from care navigators and nurses in a timely manner
• Conduct remote calls and video visits, including urgent escalation visits as needed
• Maintain accurate documentation of patient data and clinical decisions while adhering to HIPAA standards

Required Qualifications

• Graduate of an accredited school of nursing
• Minimum 2 years of direct patient care as an Advanced Practice Provider
• Current APRN license(s) in at least 10 independent practice states
• Dementia proficiency meeting at least one CMS criterion
• Proficiency with electronic health records and telehealth tools
