Job Summary
A health systems company is filling a position for a Telecommute Behavioral Health Care Coordinator Case Manager in Jonesboro.
Individual must be able to fulfill the following responsibilities:
- Regularly communicate with members about their ongoing or newly identified needs
- Develop and maintain the Plan of Care
- Assist with developing member’s self-management skills, health education and coaching
Skills and Requirements Include:
- Valid driver’s license and reliable transportation
- One year experience working with developmental or intellectually disabled and/or complex mental health conditions
- GED or High School Diploma
- Access to reliable internet
- Ability to document within care management system
- Knowledge of Microsoft Word and Excel
