Job Summary
A health insurance company has an open position for a Remote Behavioral Health Case Manager .
Candidates will be responsible for the following:
- Using appropriate screening criteria, knowledge and clinical judgment to assess member needs
- Conducting assessments to identify individual needs and develops care plan
- Monitoring and evaluating effectiveness of care plan and modifies plan as needed
Applicants must meet the following qualifications:
- Requires Master’s degree in social work, counseling, nursing or related behavioral health field
- A current, active, and unrestricted LCSW, LMFT, LMHC, RN or clinical psychologist license in the state of Indiana
- Competency in Microsoft Office applications, especially Excel
- A minimum of 3 years' clinical experience in social work, counseling with a broad range of experience
