Job Summary
A healthcare company is in need of a Remote Behavioral Health Case Manager I in Lexington.
Core Responsibilities Include:
- Performing case management telephonically and/or by home visits
- Conducting assessments to identify individual needs
- Monitoring and evaluating the effectiveness of care plan and modifies plan as needed
Qualifications for this position include:
- Local travel may be required once COVID restrictions are lifted
- Kentucky residency is required
- Current, active, and unrestricted LCSW, LPC, LMFT, LMHC, RN, or clinical psychologist license
- 3+ years of clinical experience in social work
- Must be competent in Microsoft Word, Excel, and Outlook
- Experience with community mental health resources
