Job Summary

A healthcare company is in need of a Remote Behavioral Health Case Manager I in Lexington.

Core Responsibilities Include:

• Performing case management telephonically and/or by home visits
• Conducting assessments to identify individual needs
• Monitoring and evaluating the effectiveness of care plan and modifies plan as needed

Qualifications for this position include:

• Local travel may be required once COVID restrictions are lifted
• Kentucky residency is required
• Current, active, and unrestricted LCSW, LPC, LMFT, LMHC, RN, or clinical psychologist license
• 3+ years of clinical experience in social work
• Must be competent in Microsoft Word, Excel, and Outlook
• Experience with community mental health resources
