Job Summary

A healthcare company is filling a position for a Remote Behavioral Health RN Case Manager.

Individual must be able to fulfill the following responsibilities:

• Make outbound calls to assess members' current health status
• Deliver clinical support to members across a wide array of health topics and conditions
• Identify gaps or barriers in treatment plans

Required Skills:

• Current, unrestricted RN license in the state of West Virginia
• 3+ years of clinical experience as a Registered Nurse in a hospital, acute care setting
• 1+ year of recent experience with mental health/behavioral health focus
• Must have excellent typing skills and have the ability to navigate a Windows based environment
• Certified Case Manager or the ability to obtain certification within 30 months of employment
• Ability to work independently with minimal supervision
