- Generating referrals to providers, community-based resources, and appropriate services
- Conducting assessments to identify barriers and opportunities for intervention
- Advocating, informing, and educating beneficiaries on services, self-management techniques, and health benefits
Qualifications for this position include:
- NYS RN orLCSW, LMSW, LMFT, LMHC, LPC, licensed psychologist
- Experience using Microsoft Excel
- Knowledge of InterQual and LOCADTR
