Job Summary
A company is looking for a Social Supports Service Coordinator (remote).
Key Responsibilities
- Field a high volume of phone calls and online chats
- Conduct assessments to determine individuals' goals and preferences
- Provide information, educate about available resources, and facilitate referrals
Required Qualifications, Training, and Education
- Experience in social work, case management, or care coordination
- Fluency or conversational ability in Spanish is encouraged
- Ability to document interactions in computer systems
- Experience in aging and disabilities services or long-term care is highly desired
- Availability to work within the helpline's operating hours (Monday to Friday, 7:00 am to 6:00 pm Central Time)
