Job Summary

A company is looking for a Social Supports Service Coordinator (remote).

Key Responsibilities
  • Field a high volume of phone calls and online chats
  • Conduct assessments to determine individuals' goals and preferences
  • Provide information, educate about available resources, and facilitate referrals
Required Qualifications, Training, and Education
  • Experience in social work, case management, or care coordination
  • Fluency or conversational ability in Spanish is encouraged
  • Ability to document interactions in computer systems
  • Experience in aging and disabilities services or long-term care is highly desired
  • Availability to work within the helpline's operating hours (Monday to Friday, 7:00 am to 6:00 pm Central Time)
