Job Summary

A company is looking for a Remote Bilingual Case Manager (Remote, Spanish Speaking).

Key Responsibilities
  • Maintain an ongoing caseload and promote engagement through evidence-based approaches
  • Support members with social support navigation and conduct periodic outreach for follow-ups
  • Collaborate with care management teams and assist individuals in connecting to community supports
Required Qualifications
  • 1+ years of general work experience (Case Management preferred)
  • Bilingual in English and Spanish
  • Strong understanding of cultural fluency and high degree of empathy
  • Organizational skills and ability to work collaboratively in a multidisciplinary team
  • Valid driver's license and auto liability insurance
