Job Summary
A company is looking for a Remote Bilingual Case Manager (Remote, Spanish Speaking).
Key Responsibilities
- Maintain an ongoing caseload and promote engagement through evidence-based approaches
- Support members with social support navigation and conduct periodic outreach for follow-ups
- Collaborate with care management teams and assist individuals in connecting to community supports
Required Qualifications
- 1+ years of general work experience (Case Management preferred)
- Bilingual in English and Spanish
- Strong understanding of cultural fluency and high degree of empathy
- Organizational skills and ability to work collaboratively in a multidisciplinary team
- Valid driver's license and auto liability insurance
