Job Summary

An accountable care organization is filling a position for a Remote Bilingual Korean Care Organization LPN or RN in New York City.

Core Responsibilities of this position include:

• Serving as the contact point, advocate, and informational resource
• Identifying care needs for patients and communicating daily to the care team
• Providing ongoing contact with patients

Qualifications Include:

• Experienced Licensed Practical Nurse or Registered Nurse
• Ability to verify patients' information, ensuring information and medical records are accurate
• Ability to schedule appointments
• Ability to authorize prescription refills as directed by care team
• Highly organized with the ability to keep accurate notes and records
• Ability to work productively, either independently or with other individuals
