Job Summary
A healthcare company needs applicants for an opening for a Remote Bilingual LPN Telephonic Care Coordinator.
Core Responsibilities Include:
- Conducting monthly phone calls with patients, practices, pharmacies, etc
- Creating and/or revising a personalized care plan for each patient you call on, in coordination with the practices and providers you support
- Working to identify social determinants of health, gaps in care, and eligibility for assistance or other referral services
Skills and Requirements Include:
- Exceptional verbal and writing skills in Spanish and English, including accurate spelling and grammar
- 1-2 years healthcare experience as a Licensed Practical/Vocational Nurse with an active Compact license participating state
- Technology experience
- Be highly professional on the phone
- Be able to maintain a compassionate and engaging tone
- Possess quick thinking skills to respond live to patient questions and needs
