Remote Bilingual Nurse Coordinator

Job Summary

Providing care coordination for Medicaid and Medicare members, the full-time Remote Bilingual Nurse Coordinator will build trusted relationships, assess member needs, and facilitate access to care while working from a dedicated home office in the Central Time Zone.

Key Responsibilities

• Conduct outreach to educate members about program benefits and assess their status to develop personalized care plans
• Manage a caseload of members, ensuring timely communication and coordination of care with providers
• Document actions taken in a care management platform and identify needs related to Social Determinants of Health

Required Qualifications

• Valid and unrestricted Licensed Practical Nurse (LPN) or Registered Nurse (RN) license, multi-state compact preferred
• Fluency in Spanish and English
• Minimum of 2 years of experience in care coordination or related fields
• 1+ years of remote work experience
• Experience working with Medicaid and/or Dual Eligible patients
