Job Summary

A remote patient-monitoring solutions company is searching for a person to fill their position for a Remote Bilingual Registered Nurse Care Manager in Riverside.

Must be able to:

• Conduct assessments and evaluate biometric and IVR data with trend analysis
• Collaborate with the patient and their care team, both internally and externally
• Utilize the case management process to set priorities, plan, organize and implement interventions

Qualifications Include:

• RN licensure in California
• Minimum (5) years of broad clinical experience
• Strong computer skills
• Minimum of (3) years of experience in home care, care coordination and transitions or care, case management, telehealth or related specialty fields, such as pulmonary, endocrine, cardiac, etc
• Nursing degree from an accredited college or university
