A healthcare company has an open position for a Remote Bilingual Spanish CCM Registered Nurse.
Core Responsibilities of this position include:
- Planning and conducting intervention opportunity evaluations, respond to urgent alerts and remote patient monitoring alerts as needed to help drive high quality care at a lower cost
- Developing and updating care plans for members while keeping a close eye on caregiver support
Applicants must meet the following qualifications:
- Graduated from an accredited nursing program
- Current NY RN License in good standing
- 2+ years of experience in a fast-paced health services organization
