Job Summary
An insurance company has an open position for a Remote Bilingual Spanish English Community Health Worker in the Fort Myers Area.
Core Responsibilities of this position include:
- Keeping member actively engaged with their primary physician
- Creating a positive experience and relationship with the member
- Locating members who need guidance
Skills and Requirements Include:
- Travel will be up to 50% of the time local
- 3 years of experience working within the community Health setting in a Healthcare role
- Bilingual in English and Spanish
- 1 years of outbound patient / member outreach experience, both telephonic and field-based / in-person
- Experience and familiarity with the community organizations / resources available within the community or counties specified
- Experience with both telephonic and in-person, proactive patient outreach
