Job Summary
A health management company is searching for a person to fill their position for a Remote Bilingual Spanish Health Coach.
Core Responsibilities of this position include:
- Assisting members to obtain a Medical Home and providing instruction on appropriate use of the Medical Home
- Assisting members in overcoming transportation barriers to obtaining medical care
- Informing members about available community resources and assist members in utilizing the resources
Applicants must meet the following qualifications:
- Health Coach certification from an accredited professional health coach training program
- Professional Certification, NSHC CHC)
- Minimum of two years' experience as a Health Coach
- Experience with personal computer using Windows operating system
- Ability to motivate members to make critical health changes in their lives
- Ability to resolve conflicts with members and overcome their objections about their health and behaviors
