Job Summary

A hospital and healthcare services company needs applicants for an opening for a Remote Bilingual Spanish Lead Health Care Manager.

Individual must be able to fulfill the following responsibilities:

• Use relationship-based strategies to engage members in care
• Support members with social support navigation
• Conduct periodic telephonic outreach

Required Skills:

• Have quiet and HIPAA-compliant at-home work environment with reliable Internet connection
• 2+ years of experience with community engagement, patient navigation or social work
• Knowledge of community resources and behavioral health supports in San Bernardino County
• Demonstrated professional or personal lived experience
• Strong understanding of cultural fluency
• Strong technical skills and comfort with technology innovation
