Job Summary
A health insurance company has an open position for a Remote Bilingual Spanish Long Term Services and Support Service Coordinator in Miami.
Core Responsibilities of this position include:
- Perform telephonic or face-to-face history and program needs assessments using a supplied tool
- Identify members with potential clinical health care need using a supplied tool
- Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities
Must meet the following requirements for consideration:
- Travel Required
- Bilingual English/Spanish
- 2 years of experience working with a social work agency
- OR any combination of education and experience, which would provide an equivalent background
- Experience working the HIV/AIDS and Dementia member population
