Job Summary
A healthcare services provider is filling a position for a Remote Bilingual Spanish Nurse Case Manager .
Core Responsibilities of this position include:
- Developing and managing the overall long-term health care plan for patients
- Offering education and guidance to patients and their families as they navigate complex medical decisions
- Acting as a liaison between patients and their insurance providers
Must meet the following requirements for consideration:
- Current Registered Nurse (RN) compact licensure
- RN License in MN and CA
- 5 years prior TCM Experience/CCM certification
- Strong team and collaborative skills
- Excellent organization skills and ability to meet deadlines
- Excellent communication skills- written, verbal and interpersonal
