Remote Bilingual Spanish Telephonic Registered Nurse Case Manager

Job Summary

A health services company is in need of a Remote Telephonic Spanish Registered Nurse Case Manager.

Candidates will be responsible for the following:

• Serving as primary care manager for members identified for the Serious Illness program
• Preparing member/caregiver to understand and identify signs of functional decline
• Educate and assist member with advanced care planning, including establishing and end-of-life treatment plan

Applicants must meet the following qualifications:

• A Registered Nurse that must hold an unrestricted license in their state of residence
• Associate degree in nursing
• 3+ years clinical experience required with at least 1 year of experience in critical care, ICU, CCU, Emergency
• 2+ years of case management experience in one: health plan, hospice, or hospital setting
• Bilingual, Spanish and English Required
• Have multi-state/compact privileges and have the ability to be licensed in all non-compact states
