Job Summary

A health services company is in need of a Remote Telephonic Spanish Registered Nurse Case Manager.

Candidates will be responsible for the following:

Serving as primary care manager for members identified for the Serious Illness program
Preparing member/caregiver to understand and identify signs of functional decline
Educate and assist member with advanced care planning, including establishing and end-of-life treatment plan

Applicants must meet the following qualifications:

A Registered Nurse that must hold an unrestricted license in their state of residence
Associate degree in nursing
3+ years clinical experience required with at least 1 year of experience in critical care, ICU, CCU, Emergency
2+ years of case management experience in one: health plan, hospice, or hospital setting
Bilingual, Spanish and English Required
Have multi-state/compact privileges and have the ability to be licensed in all non-compact states
