Job Summary

A healthcare company is searching for a person to fill their position for a Telecommuting Bilingual Community Transitions Program Case Manager in Panama City.

Core Responsibilities of this position include:

  • Visiting members in the hospital to engage in post hospital program
  • Making concerted effort to find members post hospital that are difficult to reach
  • Identifying gaps or barriers in treatment plans

Must meet the following requirements for consideration:

  • Must travel to the medical center in Panama City multiple times per week
  • Bilingual in English and Spanish
  • 5+ years of experience as an RN in a clinical setting
  • Current, unrestricted RN license in Florida
  • Access to reliable transportation and ability to travel to visit members
  • Must have a designated work space
