Job Summary
A healthcare technology company is in need of a Remote Bilingual Vietnamese Chronic Care Healthcare Management Coordinator.
Individual must be able to fulfill the following responsibilities:
- Meeting production goals
- Creating and maintaining comprehensive, proprietary patient care plans
- Utilizing chronic condition specific scripts to guide conversation
Position Requirements Include:
- Medical Assistant certification or higher (LPN, RN, BSN, etc.)
- Minimum 1-year experience in homecare or clinical setting
- Minimum 1-year experience in Chronic Care Management
- Speaks and writes fluent Vietnamese and English
- Knowledge of medical terminology
- Committed to excellence in patient care and customer service
