Job Summary

Care Coordinator Responsibilities

Assists clients to locate, access, and utilize a full range of health care services, community support groups, and resources.
Assesses clients' needs and functioning, presents cases in treatment team meetings, and monitors progress as needed.
Serves as a single entry or "easy access" point to help members navigate through managed care. Applies knowledge and eliminates barriers where possible.
Interfaces with social worker and case management staff in coordinating client care.
Meets Agency client service expectations.
Monitors and maintains documentation of client services; ensures compliance with Agency, state, and federal policies and procedures, standards, and guidelines.

Care Coordinator Qualifications

High School diploma, associate degree (or higher) in clinical or medical health administration, or related field preferred
3+ years experience in medical record review, triage, customer service, prior authorizations, medical claims, and billing, assessing patient health, and/or related experience
Attention to detail, timetables, and commitment to completing tasks
Experience with Microsoft Windows and computer savvy
Must be well organized, efficient, and able to work independently and within a team
Responsible for having reliable High-Speed Cable or Fiber Optic Internet service and Internet Router in established home office
Must have Excellent Communication Skills via Phone, E-Mail, Text, Verbal, and Documentation Skills and provide 24 hours follow up to all communication
U.S. Citizenship
Prior to hiring and training able to pass a preliminary credit and background check
