Remote Care Coordinator

Job Summary

Joining the special needs program (SNP) case management team, the full-time Remote Care Coordinator will support members by addressing care coordination needs, managing referrals and authorizations, and collaborating with the RN Case Manager in a fully remote environment.

Key responsibilities

• Reaches out to members to assist with referrals, authorizations, and care coordination needs
• Creates cases and completes assessments for hospital and skilled nursing facility discharges
• Establishes relationships with members and acts as a patient advocate while managing new alerts and changes in member conditions

Required qualifications

• Minimum 1 year of experience working in Health Care IPAs and entering referrals and prior authorizations
• High School Diploma or GED; Bachelor's degree or four years of additional experience may substitute for education
• Basic computer skills, including proficiency in Microsoft Outlook, Excel, and Word
• Knowledge of Managed Care Plans and Medi-Cal
• Bilingual in Spanish or Vietnamese preferred
