Remote Care Coordinator

Job Summary

Supporting patients in the Mountain Time Zone, the full-time Remote Care Coordinator will manage patient engagement, care coordination, and resource navigation to ensure optimal health outcomes through a holistic approach to kidney care.

Key responsibilities

• Perform telephonic outreach to enroll and engage patients, serving as their primary point of contact
• Develop and implement individualized care plans in collaboration with the interdisciplinary team, facilitating seamless transitions of care
• Conduct thorough assessments to identify social determinants of health, connecting patients to necessary resources and advocating for their needs

Required qualifications

• 3 years of experience in healthcare coordination or community health work
• Experience with a team-based approach to care
• Excellent knowledge of community resources relevant to patient care
• Strong ability to prioritize tasks and adapt to changing patient needs
• Intermediate skills with MS Office Suite, including Outlook and Teams
