Job Summary

A company is looking for a Full-Time Remote Care Coordinator - CMA/RMA or LPN.

Key Responsibilities

Conduct monthly Chronic Care Management (CCM) phone calls with assigned patients
Make 30-50 outbound calls per day and complete required patient interactions
Coordinate care with provider offices, pharmacies, and community resources, while maintaining HIPAA compliance

Required Qualifications

Minimum 1-3 years of experience as a CMA/RMA, LPN, or RN
Active certification or multistate license for LPN/RN
Experience with EMR/EHR systems
Organized and reliable, able to work independently in a remote setting
Comfortable being on the phone most of the day
