A healthcare company has an open position for a Remote Care Coordinator Social Worker.

Individual must be able to fulfill the following responsibilities:

• Engage members telephonically to coordinate services, community resources and treatment needs
• Deliver a holistic approach to coordinated care based on the enrollee needs
• Identify early risk factors and conduct ongoing assessments

Required Skills:

• Bachelor’s Degree in Social Work
• Professional License in good standing for respective discipline
• 2+ years professional experience in a social work position with geriatric adults/Medicare population
