A health and wellness company has a current position open for a Remote Care Management Director.
Individual must be able to fulfill the following responsibilities:
- Develop and implement care management policies and procedures
- Oversee and develop the care management team
- Monitor and analyze data to evaluate the effectiveness of care management programs and services
Skills and Requirements Include:
- 2+ years providing direct patient care in an adult chronic disease management care setting
- 2+ years of experience in a care operations, care management, or care coordination role
- 2+ years experience in leading within a clinical team
- Experience working as a part of an interdisciplinary care team
- Clinical degree in Nursing, Nurse Practitioner, Physician Assistant, or Pharmacy
