Job Summary
A telehealth solutions company needs applicants for an opening for a Remote Care Management LVN Nurse .
Candidates will be responsible for the following:
- Cooperatively developing an integrate patient centered care plan and goals with the client/family and other providers
- Providing patient assistance through advocacy to client and family for entitlements and other community services
- Triaging and providing medical guidance as instructed by the patient’s provider
Qualifications for this position include:
- Graduate from an accredited School of Nursing
- Current license to practice as an LVN/LPN/RN with compact license
- 3 years of experience as a Care/Case Manager or Coordinator either in the homecare, inpatient, physician practice or in-home case management setting
- Experience working with Electronic Health Record (EHR)
- Demonstrates progressive proficiency with the utilization of available computer technology, including typing skills
- Bilingual in Spanish
