Job Summary
A healthcare company has a current position open for a Remote Care Management Registered Nurse in Chicago.
Candidates will be responsible for the following:
- Assisting with improvement to work processes and commits to a focus on continuous improvement of workflow processes
- Partnering with the field Nurse Practitioner(s) to quickly respond to urgent/emergent conditions and hospital admissions
- Conducting regular telephonic and/or in person follow-up with members and/or caregivers
Qualifications for this position include:
- Attends meetings, training sessions, and participates on committees as requested
- Strong knowledge of medical conditions, interventions, treatment and medications
- RN license in good standing in the applicable states
- Knowledge of clinical procedures, standards and quality control checks
- Experience working with culturally diverse populations
- Proficient computer skills
