Job Summary

A health insurance company has a current position open for a Remote Care Management Support Nurse.

Core Responsibilities Include:

• Transitioning patients from skilled nursing to the home setting
• Making decisions on moderately complex to complex issues
• Determining objectives and approaches to assignments

Skills and Requirements Include:

• Ability to conduct in-home visits
• Must have a clinical background as a LPN, RN, or LSW
• Have an active nursing or social worker license in FL without restrictions
• Minimum 2 years case management
• Experience working with Medicare and Medicaid members
