Remote Care Manager II

Job Summary

To support a growing team, the full-time Remote Care Manager II will provide operational support to Care Managers, conduct patient outreach to improve care plan compliance, and collaborate with a multidisciplinary team to develop individualized care plans for patients with opioid use disorder.

Key responsibilities

• Provide operational support by backfilling vacancies and ensuring continuity of care during staffing gaps
• Conduct proactive outreach to patients to complete missed care plans and improve engagement
• Collaborate with the care team to develop and monitor individualized care plans, addressing patient needs and barriers to treatment

Required qualifications

• High School Diploma or GED required; experience in healthcare or addiction medicine preferred
• Experience in a fast-paced environment servicing multiple clients is preferred
• Knowledge of compliant documentation practices is essential
• Experience with building individualized care plans is a plus
• Lived experience with addiction is a plus
