Job Summary

A healthcare company needs applicants for an opening for a Remote Care Manager in Cerritos.

Individual must be able to fulfill the following responsibilities:

  • Implement a comprehensive clinical case management plan for each patient
  • Analyze patient variances from the plan and initiate the appropriate steps to resolve variances
  • Perform telephonic, fax and/or on-site reviews with skilled nursing facilities, home health agencies, etc

Skills and Requirements Include:

  • Ability to perform on-site reviews
  • Current unrestricted LVN, LPN or RN license in applicable state(s)
  • Minimum of 2 years of experience
  • OR equivalent combination of education and experience
