Remote Care Manager, RN

Job Summary

A company is looking for a Care Manager, Registered Nurse (Remote).

Key Responsibilities

• Engage with members, caregivers, and providers to develop and implement comprehensive care plans
• Assess ongoing care needs and progress, making necessary revisions to care plans
• Coordinate patient education and facilitate referrals to community resources based on member needs

Required Qualifications

• Current multi-state compact Registered Nurse licensure; BSN preferred
• 3-5 years of varied clinical experience, with telephonic Case Management experience preferred
• Proficiency in electronic medical records and Microsoft Office applications
• Training in motivational interviewing preferred
• Ability to work from a home office that meets HIPAA requirements
