Job Summary

A health insurance company is filling a position for a Remote Care Model Nurse Case Manager I.

Individual must be able to fulfill the following responsibilities:

  • Coordinate internal and external resources to meet identified needs
  • Perform care management within the scope of licensure for members
  • Ensure member access to services appropriate to their health needs

Position Requirements Include:

  • Must live within 60 miles or 1 hour of local office
  • 3 years of clinical RN experience
  • BA/BS in a health related field
  • Or any combination of education and experience
  • Experience with electronicdocumentation
