Job Summary
A medical company is filling a position for a Remote Care Navigation Nurse Coordinator in Jersey City.
Individual must be able to fulfill the following responsibilities:
- Assess patient condition, in conjunction with clinic staff, such as the discovery of unreported medical
- Facilitate the coordination of care for patients assigned to the Care Navigation unit
- Maintain and update related software/tracking logs of patients in the Care Navigation Program
Must meet the following requirements for consideration:
- Must be able to perform site visits to clinics, medical offices, hospitals and/or patient homes are required
- Any relevant experience
- Must be comfortable traveling remotely
