Job Summary

A company is looking for a Care Navigation Nurse to provide patient-centered support and advocacy for members navigating complex health journeys.

Key Responsibilities

• Deliver coordinated virtual Care Navigation to improve member health outcomes and satisfaction
• Conduct comprehensive clinical assessments and develop tailored care plans for high-risk individuals
• Assist members in navigating the healthcare system and provide targeted interventions to overcome barriers to health

Required Qualifications

• Active RN license in a compact state with the ability to obtain licensure in other states
• 2+ years of experience in case management for a health plan or TPA and 2+ years in a clinical setting
• Experience across multiple specialties such as med/surg, oncology, and behavioral health
• Competence in using multiple computer/medical record systems, preferably Salesforce and Google Workspace
• Experience in telephonic care management without direct patient contact
