Job Summary
Experienced in navigating VA benefits, the full-time Remote Care Navigator will assist Veterans in accessing healthcare, benefits, and community resources while also enhancing care coordination workflows and supporting the broader care team remotely.
Key responsibilities
- Guide Veterans through healthcare and benefits navigation, addressing complex cases and referrals
- Collaborate with care coordinators to improve workflows and identify program gaps
- Contribute to training and development of the care coordination team to enhance understanding of VA resources
Required qualifications
- 5+ years of experience in clinical social work, care coordination, or a related field with progressive program management
- Direct experience working with Veterans and/or within the VA system
- 1+ years of successful remote work experience
- Knowledge of VA healthcare, benefits, and resources
- Experience with case documentation in care management or electronic health record systems
