Remote Care Navigator
Job Summary
To support a growing Veteran-focused care coordination program, the full-time Remote Care Navigator will assist Veterans in navigating healthcare, benefits, and community resources while also improving workflows and training within the team.
Key responsibilities
- Guide Veterans in understanding and accessing available healthcare and benefits resources
- Manage complex cases and provide direct support to Veterans as needed
- Develop and enhance Veteran-specific care coordination workflows and training resources
Required qualifications
- 5+ years of experience in clinical social work, care coordination, or a similar field with progressive program management experience
- Direct experience working with Veterans and/or within the VA system
- 1+ years of experience in a remote work environment
- Knowledge of VA healthcare, Veteran benefits, and social determinants of health
- Experience documenting cases within a care management or electronic health record system
