Job Summary
Experienced in navigating the VA system, the full-time Remote Care Navigator will assist Veterans in accessing healthcare, benefits, and community resources while also supporting the care team in managing complex cases remotely.
Key responsibilities
- Guide Veterans in understanding and accessing available resources, including VA healthcare and benefits
- Support the care team by managing complex cases and providing direct assistance when needed
- Contribute to the development of Veteran-specific workflows and identify opportunities for program improvement
Required qualifications
- 5+ years of experience in clinical social work, care coordination, or a similar role with progressive program management experience
- Direct experience working with Veterans and/or within the VA system
- 1+ years of experience working successfully in a remote environment
- Knowledge of VA healthcare, benefits, and behavioral health services
- Experience documenting cases within a care management or electronic health record system
