Key responsibilities
- Guide Veterans in accessing healthcare, benefits, and community resources
- Support care coordinators by improving workflows and identifying program gaps
- Document cases and provide direct support for complex Veteran situations
Required qualifications
- 5+ years of experience in care coordination or a related field
- Direct experience working with Veterans or within the VA system
- 1+ years of experience in a remote work environment
- Knowledge of VA healthcare, benefits, and social determinants of health
- Experience with case documentation in care management systems
