Job Summary
A healthcare company is in need of a Remote Care Navigator Registered Nurse.
Core Responsibilities Include:
- Assisting in care coordination and care management services for all identified patients
- Assuring positive patient outcomes and care coordination
- Notifying the Team Lead of changes in patient status, medications, patient level of care, new problem identification, etc
Must meet the following requirements for consideration:
- Registered Nurse (RN) in the state of Connecticut required
- Must be proficient in Microsoft Office: Outlook, Word, Excel, PowerPoint
- Strong interpersonal, written communication and relationship skills
- Ability to work in a fast-paced environment with changing priorities
- Demonstrated organizational skills
