Job Summary
Experienced in navigating the VA system, the full-time Remote Care Navigator - VA Experience will assist Veterans with healthcare, benefits, and community resources while also enhancing care coordination workflows and supporting team training remotely.
Key responsibilities
- Guide Veterans in understanding and accessing available healthcare and benefits resources
- Support complex Veteran cases by determining appropriate referrals and escalations
- Develop and improve Veteran-specific care coordination workflows and team training initiatives
Required qualifications
- 5+ years of experience in care coordination, care navigation, or a related field with progressive program management experience
- Direct experience working with Veterans and/or within the VA system
- 1+ years of experience in a remote work environment
- Knowledge of VA healthcare, Veteran benefits, and behavioral health resources
- Experience documenting cases within a care management or electronic health record system
