Job Summary

Delivering high-quality nursing care remotely, the full-time Remote Care Nurse II will conduct patient assessments, develop care plans, and coordinate with healthcare teams to optimize patient outcomes through telehealth technologies.

Key responsibilities:

• Conduct thorough remote patient assessments via phone or video conferencing to evaluate health status and identify care needs
• Develop, implement, and update individualized care plans in collaboration with patients and healthcare teams
• Monitor patient progress and adherence to treatment plans, providing education and guidance to promote self-care and disease management

Required qualifications:

• Current and unrestricted Registered Nurse (RN) license in the United States
• Minimum of two years of clinical nursing experience, preferably in case management, chronic disease management, or telehealth
• Proficiency with electronic health record (EHR) systems and telehealth technology platforms
• Ability to work independently in a remote environment while managing multiple patient cases efficiently
• Strong communication skills to effectively engage with patients and interdisciplinary teams remotely
